physical therapy evaluation

If your first appointment at NDBC is a physical therapy evaluation with one of our physical therapists, you will need to collect the items listed below before scheduling your first appointment. During your appointment, our therapist will ask you questions regarding your dizziness or balance problem and will ask you to perform certain tasks to try and diagnose your problem. After you have met with the therapist, you will then have a preliminary balance test on our Smart EquiTest System to establish a starting point for your rehabilitation program.

The clinic receptionist will be calling you regarding your appointment and can help answer any of your questions and concerns. Please read the brochure and instructions carefully, failure to do so may result in the re-scheduling/canceling of your appointment.


Before scheduling your physical therapy appointment, these items need to be collected:

1. Referral from a physician
We require all patients to have a physician referral prior to any treatment at NDBC. If you need a referral form to give to your physician for signature, you can print one in the patient forms section of this website.

If you don't have a referral and can't obtain one, call the clinic you would like to schedule your appointment at and they can try to obtain one for you. If your individual insurance company doesn't require a referral to be seen at a specialty clinic and you choose not to get one, you will be responsible for any charges your insurance company doesn't pay according to the terms and conditions of our financial policy.

2. Insurance information
Please locate your insurance card and health insurance information. If you cannot locate your insurance card, you’ll need to obtain your health insurance carrier information, policy number, and contact phone number to verify your benefits. Please also bring your insurance card with you to your first appointment so we can verify everything is correct.

If you are under the age of 65, you must also verify your own health insurance benefits before coming into NDBC. We have a pre-assembled packet of information for you in the patient forms section to make this easy for you. You can dowloand those forms here.

3. Health history forms
For your convenience we have also made the necessary medical history forms available for download/printing here on our web site. You are encouraged to complete these forms at home before your first appointment to minimize your office wait time and to maximize your evaluation time with the physical therapist.

4. Patient medical records
In order to provide you with the best care, we ask that if you have any medical records regarding your dizziness or balance problem, please have your primary care doctor send them to our clinic prior to your initial appointment. This is not required for you to be treated at NDBC, but aids our physical therapist in evaluating your condition. This includes past ENGs, VNGs, EMGs, MRIs, CT scans, hearing tests, or any other related studies. If you don’t know how to obtain or send your medical records, call our office before your appointment and we will be glad to help locate them for you.

After your have collected your information, you are ready to schedule your first appointment.
